There's a o-ring in there, it can and does fail. On top of the mating point between parts of the QD, having a QD system adds one more barb-to-tubing connection. Each connection is a potential point of failure.

That's beyond unacceptable on the part of your instructor. It does however, highlight the importance of swimming side by side with a buddy, versus following single file. I see a lot of single file buddy teams out there... With that said, well done staying calm and catching up.

It really helps to prime the tube on the surface, where resistance is least. That should help with the back-pressure somewhat, since having the line filled with liquid will tend to ensure that any small kinks are forced out as soon as you start to pee.
It can be tricky to do when boat diving...

I haven't been diving with it this year, but did a swim test of the new wrist seal (~15 minutes with solid squeeze) and didn't find any leaks.
