As with nearly all underwater deaths the official cause of death was drowning (as per info from closely related). With an underlining heart condition.

I asked my relatives in the area, one of whom is very much in a position to know if there was more to it than was reported, and both said that all they heard was what was immediately reported--he had died of a heart attack while doing a training exercise. That is not an official statement, of course, but nothing else came out in the months that followed.
