Pin spring straps for Jet Fins

Side cutters (for removing a broken pin strap)

Vise grips (for pressing new spring straps into the holes)

Spare mask

At least 2 extra mouthpieces (I take these with me on the dive, I've mastered the art of replacing mouthpieces underwater :) )

Zip ties (for securing line where there's nothing solid to wrap around during penetration dives, and also for attaching miscellaneous gear.)

Shears (I take these on the dives to remove zip ties)

Various length and types of hoses

Extra diaphragms

O rings of just about every size imaginable

Extra slates and pencils

A second copy of my log book that I've spent about 3 months painstakingly copying, so that I can keep one in my dive bag and one to carry around, so that I'm never without one.

A second copy of every c-card I own

Extra gate snaps for my primary reg

Crescent wrenches for hose attachment and reattachment

Allen keys or various sizes

Swiss army knife

Extra DIN-Yoke adapter

Spare straps for everything

Spare webbing for my harness

lighter to remelt the nylon

Pliers

The kit has taken quite a while to assemble. Copying the log book, 219 dives was the hardest thing to do

My SAD kit generally echos everyone else's above lists but more focus on 'essentials' and less 'medication.
It also usually ends up being used to save someone else's dive, or patch up a hired kayak.

1st stage plugs
o-rings
lighter
DAN/Dive Cards
Pencil
Silicon grease
Pre cut cave line
Yoke-Din If I know the hire cylinders are yoke
Allen Keys
Small adjustable spanner/wrench
Bungee
Cable ties
mouthpiece
spare mask*
No longer carry mask or fin straps as I have a spare mask, aaand my finstraps are unlikely to break, if they do... cable ties and bungee!
pocket mask (life saving mask)
spare diaphragm**

*on the dive in my pocket
**thanks for the suggestion; having had a hole in my diaphragm once before I should really get a spare

Nic

Spare everything, even regs, drysuits, can lights, the whole business :)

Helps to dive out of a truck where you can pack in an entire diveshop!

Mask case? No, no... our SAD kit is the same size as our reg bags. Okay, here we go...
Side pocket 1 -- ziploc bags, two adjustable wrenches, some weird multi-tool
Side pocket 2 -- two plastic magnifying sheets, good for checking the small stuff

Inside mesh pocket -- fin straps, two with clips for quick delivery. We stopped carrying a mask strap when we switched to soft wide neoprene straps.
Left pocket 3 -- computer manual, batteries, o rings, underwater earplugs (Richard), zipties, Benedril, Bonine (thanks for reminding me to toss the old meds)
Left pocket 4 -- camera manual, buddy line (biners and paracord), Neosporin, Swimmer's Ear, Sea Gold, extra biners, cord, safety clips, misc tiny stuff in mini zips for which I have no explanation -- Richard's stuff

Center pocket 5 -- two allen wrench sets
Center pocket 6 -- multi-tool, sewing kit, old and easy-to-clip snorkle keepers, split rings
Center pocket 7 -- old SAD kit mouthpiece, female gear clip with webbing strap
Center pocket 8 -- more o rings and small computer manual

Right pocket 9 -- three waterproof Ned DeLoach reef fish and creature guides, nail clippers, silicone grease, Zooke wax
Right pocket 10 -- zipties, silver Sharpie and white paint markers to I.D. new gear, small rubber hose with cord and cord lock (from a flashlight)

Either the 100-ft line or the 50-ft line is wrapped on a holder and in the SAD kit. The other is tied to the dive flag.
The SAD kit goes everywhere with us and it does get used, normally after we get back and have time to tinker with gear. Onsite we've used the pre-clipped fin straps and the Bonine. Sometimes we needed the extra Sea Gold or Swimmer's Ear if we ran out of what was in our mask cases.

Sex bolts ???!!! In front of the kids?!

I have O rings, spare fin and mask straps, zip ties, spare mouth piece, silicone grease, and folding dive tool. I keep paint marker just in case need mark gear on a boat. Bonine now after episode seasickness. And extra plastic bags, I put hot water in them on cold water dives to help my daughter stay warm in between dives. Cheap hot water bottle and works better than chemical hand warmers.

On boats I always carry spare mask.

One thing I forgot to mention...when taking a rescue class years ago, we had a bystander or fellow dive buddy "make a call to 911" for help. Of course there was no real call, but we all realized that some spare change for a pay phone would be handy to have at all times. I find dimes and quarters good substitute screwdrivers on occasion too.
