Diving Log 4.0 (Print to 5.5 x 8.5)

Has anyone created a Diving Log 4.0 report file to print logs horizontally 2 on a page so they can be cut to 5.5 x 8.5 and punched for a standard logbook? If so, I would love a copy of the file if you dont mind sharing.

I have managed to create the report format. It will print 2 pages to a sheet that can them be cut to 5.5 x 8.5 and punched for your logbook. If anyone would like to have it just send me a PM. I tried attaching it to this but it would not allow it even though I saved it as a zip file and it was very small.
