Insurance release form SSI / PADI

I'm cross posting in hopes of that SSIGuy will have some idea ;)

In a nut shell I'm looking for, or asking for, a universal release form that any dive agencies insurance carrier would accept. My main interest today is SSI and PADI for the try scuba program and BSA award

You could try the World Recreational Scuba Training Council (WRSTC) (http://www.wrstc.com/main.php) under the downloads section they have a generic Medical Release
