Let's assume a diver is on a liveaboard and has a suspected DCS injury. My understanding is that the diver must first obtain a doctors order and then an insurance pre-approval for the evacuation to be covered by Dive insurance?
Is this a correct interpetation?

Depends on the liveaboard. In the US, the USCG covers the cost of evacuation, as in all us taxpayers cover the cost of evacuation. I've never had DAN or Diveassure fail to cover the cost of treatment on any of my evacuees. We call DAN emergency line for every DCS case, regardless of insurance coverage. When they recommend evac, I figure they can call the insurance company themselves.....
Remember, your primary insurer covers up to their limits first for your treatment.
