I had my USD spg fail during a dive. very rapid leaking out of what appears to be an over pressure button in the back. it was in my pivot so it was hard to check underwater Has this happened to anyone?

Sounds like the HP bourdon tube inside failed and vented out of the vent plug as designed. It is a goner. If it is not very old, you might see if the mfgr will give you any credit on a new one.
 
If it's a USD Pivot, it's got a few years on it and is probably outside of warranty.
