Just curious as to what questions you would ask a potential OW instructor. I took my wife to an LDS last weekend for an intro to scuba class, and the instructor was horrible. We're now looking at other LDS's, and I'm trying to come up with some good questions to help limit the chances of a repeat of last weekend.

This list posted by Walter is about as good as it gets.

To the above I always advise to ask:
What if I need some extra time or attention on a skill or something we are working on in the classroom/cd rom portion of the class?
What if I cannot make a classroom, pool or open water session? Is there a charge for the make up and if so, how much?

Walter's list is a great one. I'd like to emphasize asking about the instructor's own diving activities. Someone who dives actively for fun is much more likely to have a broad base of experience from which to draw, both in instruction and in illustrations. I have seen people who marched through all their recreational classes, straight into DM and instructor classes, and have done almost no "fun" diving for themselves at all. They have little experience with which to evaluate the teaching program they were given in their IE, and therefore little ability to be creative or flexible. I would avoid such people.
