Not all, but most that have an integrated hood. It helps the upper body suit stay extremely tight and streamlined for minimal drag. Freedivers tend towards these types of suits. Of course you can wear a common full wetsuit which may not stretch quite as tight. Wetsuit fit is critical for warmth.

In my professional opinion, Johnoly is right, but also note that most of the spearfishing suits are lined inside with open cell slickskin, a material that is specialized for hunting wetsuits for extra warmth and buoyancy. This material has an amazing fit (like a second skin),
but it's too difficult for one to put on/take off the suit. So it needs to be two-pieces, and that's why the beaver-tale.
