Like I said calling would be better, since you did want to use the number on the form (800) 647-0605 ext 761. They just got back from Dema and Thanksgiving was last week.
Doug got promoted this year and they have not hired a replacement for him.
 
Have patience. You PM'ed him during DEMA... Immediately followed by a shortened Thanksgiving week.
 
My response is still valid. Give him a day or two to catch up from DEMA and the long weekend.
