I dive with a CDX-5 first stage. I had a blowout of something internal to the first stage when I turned on the air to the regulator. It started with a bang and a rush of air sound similar to a tank o ring failure.

In this case, the air leak was from inside the housing, not from the plugs or hose connections.

I will get it serviced, but looking for info.

What is likely point of failure? What would have happened at depth?

Can you be clearer about where the air was leaking from? Was it from the yoke area, the LP end of the body (housing), or the HP end of the body.
 
It seemed to be coming from the adjuster cup area on the side of the reg. Looking at the parts diagram, I think that would be low pressure side.

I don't do maintenence on regs, but wonder how this failure would have affected air supply.
