Spain: medical certificate

My 16 year old son and I are going diving in Spain this summer. I was told that we will need medical certificates. I was also told that we could have it done there, but I'd rather take care of this before we leave. Do I need a special doctor? Where do I get the paperwork that the doctor needs to fill out? Thanks in advance for any help you can give me.

You may obtain the required medical certificate form by writing DAN Europe at mail@daneurope.org.

It is pretty basic and the questions can be answered by your son's personal physician.
