Can you narrow down areas - Malaysia and Philippines have a lot of dive spots. I liked Sabang in the Philippines with South Sea Divers and would highly recommend them. Malaysia...depends on your budget - from Kapalai at Sipidan (big budget) to Seaventures Oil Rig also in the sipidan/mabul area (low budget) to Perhentians, Tioman, Derawan to name just a few - lots and lots of areas.

Cambodia - try Scuba Nation Diving Centre in Sihanoukville. I was happy with the service.
 
In the Philippines we are thinking of flying into Cebu and have heard good things about Malapascu and Palawan. Are there any safety concerns here - we haven't found any.

In Malaysia we are looking for mid price, nice diving and a nice beach/resort to hang out at that also is near some good land tour locations.

Depending on your experience level and what quality of diving keeps you happy, it may be that peninsular Malaysia wont excite you much in terms of diving.

In Malaysia the best diving is in Sabah (the bit closest to the Philippines). Sipadan resorts are usually the first place people look. Its a an awesome destination - small island - tonnes of stuff: sharks, turtles, thousands of jacks, barracudas, bumphead parrotfish etc and loads of small stuff at Mabul and Kapalai.

There is also loads to do in Sabah: Mountain, river, jungle, culture etc.
 
You would be incredibly lucky to find big creature encounters in Cambodia. Philippines or Sipidan you would stand a much better chance.
 
Fly into Cebu
Thresher shark at Malapascua.
Whaleshark at Oslob(you are advised to read the background of the dive offered because some people do not like it)!

Moalboal(school of sardines) is also very nice.
All these three places are on the island of Cebu. But travelling in Philippines or in SE Asia do require patience and time.
