Documented Equipment Failures

I must have too much time on my hands today, but anyway... I've been bothered by talk about equipment failures that people have "heard of." Is there a central repository for actual documented reports vice the standard 2nd and 3rd hand stuff that seems to always get posted?

Not a good sign--the silence is not encouraging. How do we get access to the manufacturers malfunction/failure data? Are there websites out there? Should we start a subcategory under Scuba Equipment on this board to list the ones we know of with certainty--like my former dive computer?
