I have unable to identify this reg/tank combo and more importantly, how to fill it.

Unless I am mistaken, what you have is an aircraft bailout bottle make by AL for the military.....I may have the fill adapter. :)

Does the adapter connect to another scuba tank?

Yea, has a yoke on one side and a 3/8-24 threaded post with an oring on the other side.....I bought thinking I could use it as part of a transfill whip.....I am looking for the manuals I saw a while back.....stay tuned...

How cool is that

If you have some of the bits and pieces to fill something like that like a yoke din reg or a hose you can always get the rest of the bits and pieces from the hydraulic shop
