I have been trying for months. They evidently did not convert computer records of NASDS certified divers very well. I called SSI-USA (number is on their website www.ssiusa.com) and they faxed me a form to complete.
However, nothing is easy and since I wanted them to reissue my card in my maiden name, I ended up having to re-visit the shop who originally certified me and fill out another form, plus have my picture taken?
That was 8 weeks ago and nothing yet ;-0
