    Visual Causal Analysis of Incidents for Communication

    Attached is a Visual Cause & Effect Diagram for analysis of Incidents. I am about to write and present some technical discussions at a Reliability Conference please comment regarding how well this communicates incidents.

    The information used in this analysis comes from another thread posted on the forums.
