I probably will arrive Maya port around 4:30 to 5 pm.

What time is the last ferry from Maya port to Malapascua?

If I miss the last ferry, is it easy to hire a private boat to go to Malapascua immediately?
How much?

Last boat varies depending on if there are passengers or not. But any time after 4pm can be difficult.

Private boat can be from P800 to P2000 depending how much they think they can get out of you. Plus extra if you have to ride a small boat at either end to get to the beach in low tide.
 
Yes ranking right up there with the worst experience I have in the Philippines compared with Manila Taxi driver fare to Terminal 1 (International) from Makati. I usually try to get to Maya before 12 noon to wait out the Ferry tycoons. If you do pay a "special" fare they will still wait for additional passengers to board a boat you have already supposedly paid for in full but for which they will collect. Fortunately the return trip is mostly very straight forward simple exchange of the standard water ferry fare. Your best approach is to act as if you could care less and say you will simply wait for the next scheduled boat. They will will usually come back with a lower offer. Whether you truly need to get to Malapascua that night well only you can be the judge as to what it is worth to you. As Matt says the if it is low tide and it is necessary to shuttle you out to the Ferry banka (local out rigger boat) it will be another 20 pesos for you and for each handled piece of luggage usually.
