1)YMCA/Scuba Diver, PADI/AOW, Rescue, and a couple specialties.
2)US
3) I forget, many hundreds all together
4)? 40-50X a year, sometimes more, sometimes less

Not that I consider a certification a license. Do NOT give the politicians any more ideas please.

Agencies do not issue licenses, they issue certifications. There is a big difference between a license and a certification.

I have Open Water certifications from YMCA and PADI. Other certifications I have are: Silver Star Advance (not AOW) certification from YMCA, SCUBA Livesaving and Accident Management (SLAM) certification from YMCA, Dive Master from YMCA, Assistant Instructor from YMCA Silver Star Instructor from YMCA, various specialty Instructors from YMCA, Gold Star Instructor from YMCA, Three Star Instructor from CMAS, Institude Director from YMCA, Nitrox from NAUI, Instructor from SEI.

I paid for instruction, not for certifications.

4. how frequent do you dive?
I usually make between 100 - 150 dives per year.

Thanks for all those detailed answers, it's great and much helpful.
For those who want to know more about this survey: we are studying about international sport organizations (profit and non profit) and need write about one sport. Since I have good friends who dive I decided to write about all those certification agencies. I would like to come up with a table of answers from all continents and run small statistic in Excel, nothing fancy. No names or any personal information is of any interest. If the final result is of interest for this forum I will be happy to post it.
The reason for using License instead of certification is because English is not my first language.

More answers are welcome.
Any idea about other relevant forums to get more answers?
thanks
Michal
