I would blame dive op a little. They should have had a person looking for anyone on the surface of water while the dive takes place. If the divers really had a colorful floating device (as the article says), I guess the SMB, then they should have been well seen from afar.

I bet dive op did not pay attention.

Don't know anything more than the printed story. if anyone learns any facts (dive op....how they lost contact, etc....) please post it.
