Anyone know of a dive guide book or site that has maps and good info on dive sites north of Phuket? Of course it's fairly easy to find the Similan/Surin info, but is there anything for Burma at all?

No, for Burma not available afaik. Slowly slowly there are some DC's starting in Burma, they might be able to help you out but I think also there this information is not yet available.

There are dive guide books available for Similans, but not yet for Burma Banks that we're aware of. Try websites like the following for Similans/Surin info books:
