I originally purchased and started using my custom USIA dry suit in 2003.

This weekend after 4 glorious years of absolutely no serious problems, I think I have worn a small hole in my neck seal.

I patched it with inner tube kit, and cemented it good on the inside of the lower neck part to make the New Years Day dive, but know that it is on its last leg, since I can see two more little worn areas that may be next.

Any suggestions on what to do, or any other repair ideas?

I also emailed the manufacturer for their suggestions and have not heard from them. That could be due to the HOliday season. We will give them a couple more weeks to see if they are going to provide clear suggestions on a couple thousand dollar dry suit.

I would give them a call. For whatever reason some companies do well with email and other do will with phone calls. My limited experience has been that USIA is better with phone calls. YMMV.

They contacted me today. :D and here is an excerpt from the email thread:

I did this and found out more information today. It seems that the factory can do the repair but it will take 4 - 6 weeks turn around to complete. They provided me Aqua-World in Garland Texas number 972-240-4559 and I talked to Rick who said they would be able to do it normally in 2 weeks time for about $100 - not bad for Scuba equipment
