Not a mushroom coral. Mushrooms are soft corals. Definitely a large polyp stony (LPS). Perhaps a Pagoda Cup Coral Turbinaria Sp.?

I could be wrong, but it doesn't look healthy. Bleached and it looks like it's showing signs of tissue necrosis.

I hope not. It is very beautiful.
 
Thanks! Though the creature on my pic looks differently. Note the transparent "tentacles" with milky white tips on the sides of the stems. I haven't seen anything like this on cup corals. I did not touch it so I do not know if it is soft or sold. I could not find anything like this in Coral Index.
 
Looks like it might be a Galaxea coral.

Could be. I owned a galaxea coral before. It looked a bit different than those pics... Super aggressive coral. Definitely the meanest coral I've owned.

I know you’re kidding but they aren’t that far off. I had an Aussie hammer coral that ate a sea hare. That was an interesting day in the tank rescue department.
