I don't believe there is a reg meeting your requirements.
There are many with 5 plus 2.
 
I mean, every portplug and oring is a failure point.

What you say is true, but the risk is very very small. Also you need to think about the failure mode. If the o-ring starts to go you will get a small stream of bubbles first. So it is not life threatening.
 
If you also want to eliminate the swivel turret and still keep the end port you have a choice of an Atomic (Z I think it's called) but it has at least half a dozen LP ports. Scubapro MK1 (1 plus 1), MK8 (4 plus 2) or MK9 (5 plus 2).
You could get creative and marry a MK1 with a MK8 and end up with a 4 plus 1.
.........but why, when there are many very good swivel turret piston regs that aren't rare or vintage.

And...if its for monkey diving, I wouldn't be worrying about it.
Unless monkey diving is something else...I understand it to be straddling a pony/bailout and thats it.

I am not exactly sure what monkey diving is. Some say it is sidemount with a single bottle, others say no BC is used and others use a scooter. In any event, I am not sure why he needs a balanced piston reg with a bottom port.

Bottom port for hose routing, piston because it will be used with 50-80% o2 on occasions and pistons are easier to o2 clean. Balanced because ill also be taking it deep.

Well, then, why not just bring a tank and breathe off the valve?

Some people take this 'failure point' argument to an extreme. Just get the regulator you want and take care of it. But if you're sure you need a balanced piston with 1 HP port and 3 LP ports, get an old MK5. But there are other problems, like the turret o-ring, that might keep you up at night.

If you're willing to forgo the balanced part, a MK2 is probably about the most failure-resistant 1st stage in history, along with the conshelf. (That's a balanced diaphragm). Good luck!

I hope you will not be taking an 80% O2 mix deep, however being you are an instructor I will assume you know what you are doing. The closest first stage to what you are looking for is a Scubapro MK-21 with the tech cap. It does not have a turret but does have the bottom port. I believe it has 5 total LP ports so you will have 2 leftover.

Also a balanced regulator has nothing to do with depth. All regulators are depth compensated. Balanced means the IP will not vary with tank pressure. A MK-2 would make a good O2 regulator because it only has two internal O-rings. A sealed atomic would be your worst choice because it is sealed with grease which is NOT what you want in a high O2 environment.

Also I thought monkey diving was shallow.

I'll be using 50% for 60-30 and 80% for 30-surface. I mean deep is subjective isn't it? I guess I mean that I'll be doing dive also with the 80-120 ft range on this rig
