Piston broken through the balance chamber

Odd. I would've thought the adjustment knob braces the balance chamber end enough to prevent it being broken through.

A serious HP leak in the first stage ?
 
Where is the spring?
 
ScubaPro MK-2 first, 156 second. IP was 145. Spring is not in photo for clarity. Wondering if balance chamber had a defect..... never seen that before.

Last rebuild was 4+ years ago.

Wife who does this for a living in a Scubapro shop says that the balance chamber is FUBAR. Should be closed at one end.
