Might it be an alternative to a gagstrap for relaxing a jaw?

The flange maybe goes under the lips and seem like it would seal tighter when pulled?

No, this flange is over the lips. I`d saw such design for cold water mouthpieces, that cover lips over.

You can check this two kinds of mouthpieces:
standard, just chineese design, I had changed all of my mouthpieces to that: Tec Dive Mouthpiece

it has thick pins and your jaws stay little opened, and anyway you keep it strongly. It take out forces from you jaw muscles, and you do not need clench yout teeth.

When you go through that box of old Regs you’ll find something like this, I believe it’s for the calypso j. This sort of mouthpiece back in the day had attachments for a neck strap (maybe also in the box) the only use I see for something like this is to keep those nasty stingers off of your lips, I would just make one and save the $50.
 
This is what I have found to work for me. I no longer grip the mouthpiece with my teeth. This mouthpiece kind of hangs onto my teeth without tensing my jaw.

I've been using this type of mouthpiece for over 20 years and all my regs have them fitted, highly recommended.
 
For further discussion: the flanges are intended to go inside the mouth.

Here's the quote off their website:

"Squeeze the “Manta wings” together and insert mouthpiece into your mouth. Let wings open up against the inside of your cheeks. Once regulator is in place, you do not have to bite down on the mouthpiece. The Manta Bite can be adjusted if it is too large for your mouth by cutting off a piece of the ends to the desired fit.

Word to the wise...if you get the off-brand version of these, get the clear ones. The off-brand black ones get hard & scratchy really quickly.

I use these on all my regulators, rebreathers, everything. Love them!! I usually have 4 extra with me on a trip just in case I need one and so I can convert the uninitiated :D
 
For $10, I'd try it, for $40, kick rocks.

I use the comfobites too. The cheap ones are noticeably thinner material all around and don't work for me. The real Aqualung ones are fantastic.
