No, the adapter connects to the end of a regulator hose, not the end of a BC inflator hose.

It's called a Sherwood Shadow, and was made for a couple of decades at least.

I don't see a thread size mentioned so can we assume that a second stage will screw right into the fitting on the right?

Yup, that's what I was thinking of. I'm surprised I haven't seen one before. Plus it's probably better than having someone come up to me OOA, telling them to hold on a minute while I disconnect the BC hose, get the spare second stage out of a pocket, stick it on the hose, and hand it to them.

I just leave my regulator as/is and just grab my pony or add my 40's side mount... can see change stuff around all the time.

Excellent! Thank you! I'd looked at that adaptor before but it wasn't clear to me that's what it was for, so I thought maybe it was some "tech" thing I'm not familiar with. Now I can get my old second stages out of the "when I get enough parts" bag and use one of them.

Ah the Air2 controversy. Technical divers are against them. I still love mine. I solo dive shallow open water Florida shore dives with a 5 foot long hose passed under my right arm and around behind my head, vintage plastic backpack, and my old wing with an Air2 on it. I already own it and I see no reason not to have a power inflator that doubles as a regulator. If it started to free flow I could disconnect it and orally inflate my BC. If my primary reg started to free flow I could breathe off my Air2 while I monkeyed around with the primary. I tried a friend’s bungeed second stage but I really don’t like things around my neck, the long hose is enough there, and I’m so used to having an air2 that if I removed it in an emergency I might still try to breathe off my power inflator LOL.

Do not get into the weeds on "Failure Points"
That dead horse has been beaten into oblivion.
Dive gear is very well made these days and gear failures that happen are very rare. Improper mantainace causes the most problems . Most dive accidents are more by diver "panic" and error, lack of proper training, then the boogyman "Failure Points".

Basically an old school, use your own power inflator, Air 2 (without the mods to incorporate the reg and inflator into a single unit)...yes?
