Ohio volunteer firefighter killed while trying to drain air from alum 80, with non-functioning valve, after loosening PRD. Cylinder was not secured in a vise and HP air from PRD sent cylinder spinning uncontrollably into the engine bay. Valve was sheared off from impact with a wall at which time the cylinder became airborne glancing off a fire truck and then hitting the victim in the head. Fire department had purchased several used alum 80 cylinders and was getting them ready for new hydro-tests when accident happened.

Excuse me. I did a search in forums and didn't see a post about it. However, after reviewing the lengthy and varied speculations after the initial accident and more recently, I didn't see a reference to the official investigation made by NIOSH. I'll just leave the post up for those who may want a PDF to forward to fire departments, scuba clubs, training classes and/or post in dive shops. Safe diving and equipment maintenance to all!

The Search function can be challenging to use, I know, and today it's even challenging to find. I don't see it where it usually is. Odd.

I try to help post on new accidents in A&I, but I greatly appreciate others doing so as well. I post on a variety of things so I would not suggest checking my activities. In A&I, it's usually easy to scan recent threads about recent incidents, and using the Search feature can help if you hold your mouth just right and everything else works. I hope to see that back very soon. I'll report it as MIA.

I had a close call once with medical oxygen tanks. My favorite thrift store had some for sell, but the valves were stuck. If I hadn't read about the horrible experiences of another fellow when he forced open one, incurring severe injuries, I probably would have tried the Bigger Hammer & Bigger Wrench approach myself. Since I had, I encouraged the manager to call the fire department and ask for help is disposing hazardous tanks, she did, and they got rid of them safely.

Ok, the usual Search feature I use on specific forums is missing, but I see that there is another in the menu at the top of the page, even tho it takes more tweaking. That's another example of using Search as doing so for Firefighter and Fire Fighter may give different results.

I found the A&I thread on the accident from last year, Firefighter killed filling tanks - Ohio but the information in the link that Jimbo found was not in it, so I'll add it. Thanks again Jimbo2.
