Going back to the theme of the original post, ever since the first SARS epidemic Chinese diving customers in Asian resorts would bring their own new mouthpieces on diving holidays. And many Asian diving operators would also offer new mouthpieces to every new customer. I haven't seen this at western resorts (yet).
 
It was not a survey or case study. Actual Incidents Analyses. The original report is available on the internet to read if you search for it.

Reading through the BSAC decisions on alternative air sources makes it very clear that their decisions were primarily motivated by tort concerns.

My first LDS made their customers buy a mouthpiece and would place your mouthpiece on the primary second stage for pool and OW dives.

BSAC insures their members diving activities, and the report simply makes clear that if your diving activity doesn't adhere to BSAC recommendations and teaching and that is a contributory factor to an accident or incident, you may not be insured. It is about safety.

Note that the report makes clear that Long hoses are fine, but not hogarthian loops.

There has also been a DAN incident report on a Hogarthian loop contributing to an adverse incident.

Lol, she is too sweet not to keep, though she was pissing me off "helping" me wrap my sons b-day presents..... she wants to dive too any time I start reading my DM stuff she is instantly sleeping on it.

So I googled the definition of Hogarthian loop and came across this:

The excess length of the long hose is stored by “wrapping” it around the body. The hose passes down behind the wing on the right hand side, around the right hip, across the chest from right to left and behind the head. There is a common misconception that it goes around the neck. This is not true as it would be dangerous to have the hose fully around the neck. By having the hose on the back of the neck it id held in position but can be released by ducking the head and/or flicking the hose off the head. This method of storing the hose is known as a Hogarthian loop or it is often shortened to “Hog Loop”.

So how does BSAC teach to store excess hose?
