If I needed air and that was the only working reg within reach, It would fit my mouth just fine.

This. For example, I have an extremely strong gag reflex. Dentist visits are just miserable. ANY mouthpiece with the long sides makes me gag horribly, and the molded ones are worst of all. AL Comfobite is what I use.

I use one. Only downside is having to remold after every new crown.

If I was in an out of air situation, a molded mouthpiece would be absolutely no deterrent to me getting a breath from a working regulator. A very different response to a controlled reg swap in a pool.

I have had a Seacure mouthpiece on my primary regulator for a very long time. I have always had a primary donate system, first it was an Air 2 with a short hose primary and now it is a necklaced short hose second and a 40 inch hose primary. Are there people who should/could not be my buddy due the the gagging?

Looks like @Marie13 and @MaxBottomtime should probably not be my buddy

That’s why I would like to start with secondary donating practice at the beginning of a dive trip to make sure my octo is working properly (including purging off any water trapped in it before handing the octo to him) and vice versa, my buddy’s octo is working properly and it won’t gag me. I haven’t done this for 15 years I have been diving.

Unfortunately covid19 makes OOA practicing sketchy. So not using seacure mouthpieces or only having buddies who have tried it in the past (or immediate family members as buddies) seems like the most viable path

In an ideal world, that's wonderful. And marvelously conscientious. Bravo! I had a second stage hose go pop in a wreck many years ago. I gladly accepted the nearest diver's regulator without thought of the brand of the mouthpiece. Not my buddy either. When sh!t happens, you're unlikely to decline the offer of an air source because it's the wrong size, color or shape.

My spouse had (she has since quit diving) a crazy strong gag reflex similar to @Marie13 and has a small jaw

She "drowned" in her tech class (or GUE-F I forget which it was) when her buddy tried to donate a seacure mouthpiece with moulded teeth and long bitewings. She had to go to her own necklace while coughing and recovering from gaging. People do violently reject mouthpieces that don't fit without any real opportunity to think of the consequences like we do from the comfort of our keyboards. And its not a choice you get to make twice.

Lucky enough to have the same dive buddy for over 15 years and I know he has no COVID-19, at least we have no problem driving & hiking together for 2 weeks in September.

My octo has regular mouthbit too. We’ll see what happened during diving this weekend.
