Probably kept unscrewing until it comes loose then they found out about rule number one for working on anything either on a boat or from a bucket truck once you drop it its gone!
 
There is a locking screw to hold the knob on, if I am not mistaken. It was probably pretty funny watch a committee of novice divers (these things always happen to newbies) trying to figure out how to turn off the air to break down the tank and retrieve their regulator from the rental tank.

Aside from the usual assortment of weights, masks; the occasional fin, I found a bikini top firmly -- and I mean firmly -- attached to a decorator crab a while back . . .
