Form Required for Medical Statement - Professional Status

Hi Guys,

I have just completed my Science of Diving & Dive Guide course and I'm working towards Divemaster Professional Status.

Does anyone have a link to the correct form I need to get filled in by the Doctor. I can't seem to find it on the SSI website. I have contacted my Dive Centre and am awaiting response but just wondered if anyone knew in the meantime.

if you just did your dive guide training then your current medical should still be valid unless it is more than 12 months old or you have had a change in your medical condition

btw.....science of diving + dive guide = dive master automatically

Also, it's the same as any RSTC form. Doesn't have to have the SSI logo on it. Just FYI for the future.
