I'm a little late in responding! I have a hooded vest and I can't take it off without someone helping me - and that is the right size. I usually wear mine underneath my wetsuit instead of over...it feels warmer that way.

If you want a zippered vest, you may be able to find some with surf products. Patagonia, Prana, O'Neill, etc. If women's, Prana definitely has them, as does Patagonia, Roxy.

My wife has a 4th Element hoody vest. She can put it on herself, but I help her take it off. With a snug fit, it really works well.
 
I was just at a LDS and they had some zippered vests that I'll give a try; the owner mentioned wearing it on the outside as did someone in this thread. I just don't want to have to rely on a stranger to help me remove a non-zippered one. The LDS is also getting in some Fourth Element thermal skins that he said may provide a bit of extra warmth; he saw them at a trade show.
 
Here's an update: I ended up buying a Henderson Thermaxx 3mil at the LDS; I tried it on under my suit. Just got back from a trip, where I wore the vest under the suit. It worked great, compressing enough to keep water from flushing in and out against my trunk. My arms would get cold during some of the dives, but never my core. Easy to get in and out of, and was even able to zip up the wetsuit without gasping for breath.
 
I wore the following vest under my 5.5mm wetsuit today, in a 17C (62.6F) lake. It is 2.5mm neoprene:
