The first hole to the right looks like a bullet impact with lead splatter.... Probably sold as a war relic.
 
The one hole was drilled. It was supposed to be a UT test piece, which UT missed. We picked it up on visual. I have actually seen several holes in cylinders, particularly from CO2 service.

The one that really got our attention was the bottom blowing out the cylinder. Blew the machine safety and bent the tie down clamps and that was hydrostatic pressure. Hate to think what would happen with gas!!!
