The issue in IE is that you don't have the luxury of missing the 5 count or you fail your Rescue #7 scenario. In real life yes, you miss by a little, pack 2 breaths and continue. We are strictly talking IE here where demonstration skills are required. 1...2....3...inhale...simulate rescue...
Do they belong to an IE if they pull out a knife to cut one wearing BP/W out of a webbing? How would you feel if a fellow IE candidate pulls out a knife and gets you out of the webbing? After all one was not confused and applied proper rescue technique.

There is probably no instructor in US that would take you as a student w/o signed medical form. If your MD cleared you, you should bring the form signed by MD. It doesn't go into any details just stating that MD finds you ok to dive.
By not filling honestly (and not being MD to make qualified medical decision on it) you do not provide the dive professionals information asked. Feel free to build relationship on a lie, but instructor should have an opportunity to decide to take or not to take the risk with particular student...
You are actually exposing dive professionals to additional risk by doing so. It is a YES/NO question and if you see MD then have him sign you are clear and be done with it.
If I knew any of my students has done that I would certainly not be taking him/her under water.
I do have two YES on my form and every year I bring the form to my primary care MD and he or PA sign it just fine. It is valid for one year so I just make copies and keep few in my logbook just in case I am asked to fill in the form somewhere. Problem solved.

Basically you work for 6 months to cover $3k worth or $6k/year or way less than minimal wage. Granted it gives you good view of how much is scuba professional actually worth.
