You can also book most Thailand LOBs through a local dive shop in Phuket. Contact them through their social media and work with them to sort it out; they can also answer questions about particular boats or itineraries. They also have info on any special deals (but unlikely to be any in Nov/Dec)...
For December, the height of the high season, I'd say have something booked by September if you want a particular ship or dates. If you're more flexible and going in November, a couple months before (September again) is fine.
There is varied marine life, and a lot of macro, in Malapascue not just Threshers. But I agree, do to time constraints and diver experience it would be better to go elsewhere.
People should bare in mind that Dive Concepts is a "value" operator here in Bali. They have generally the cheapest prices and therefore attract a large and for the most part young crowd. This means that they have a lot of people passing through the dive center daily so more of a chance for...

Ambon and Lembeh are both known for their muck diving, so unless you want a double dose of it back to back you may want to reconsider this itinerary. Instead of Lembeh, maybe do resort based diving in Bunaken or Banka before heading to Ambon for your liveaboard.
A very nice trip report...should be very helpful for many people. Pretty much sums up LOS diving...good not great. Too bad your southern Andaman trip didn't include Daeng/Muang and Ko Lipe...the best diving in Thailand IMO. When I'm in the Phinisi, I just sleep on deck in the saloon or topside...
