Aqualung Legend LX First Stage Failure at depth

Wow - that has my heart rate up! Your symptoms are identical to mine. 1/2 tank, no air from first stage, slowly came back when I got back on the boat. Glad it worked out well for you. I had the issue, another person on this thread did as well, you and there is a DAN report on the same failure.

In the interest of posting the full contact story - here goes. My initial contact was in writing to the Aqua Lung Service department detailing the incident (9/3/2016) via the web site contact us page. The auto response stated I would be contacted within 5 working days. I was not contacted...

Hi. I am sorry this is so late. I haven't been on SB for a while. I have attached the response from Aqua Lung. There was a service advisory issued and I assume it was in response to this. Not sure. I now dive with a Scuba Pro regulator and I have an Aqualung Titan as a back up reg. I got...

Aqualung Legend LX First Stage Failure at depth

Sorry for so many posts. Here is a DAN incident report I just came across - "Air supply cuts off when regulator failed" Divers Alert Network, Air supply cuts off when regulator failed In comments - "This particular first stage has an Automatic Closer Device to seal the first stage when it is...

Aqualung Legend LX First Stage Failure at depth

Hello Wynner Can I make a suggestion. I had the identical issue to you. Take your 70 feet and fill in 60 feet for me and the experience is identical. I also had the pressure return very slowly after the tank/reg got back to the boat (but not while in the water). I reported the issue to Aqua...

Fellow divers, The ACD issue has nothing to do with the yoke being loose and the problem is not visible on an assembled regulator. It has nothing to do with the yoke nut being too loose to push the ACD in to allow air to flow and having air gush out of the tank because it is too loose. The...

Aqua Lung Voluntary Recall on ACD Yoke regs

I had this failure at 60 feet half way through a dive and opened a thread on it a few years back. It was an interesting discussion. I just saw the consumer safety notice today. I did mail Aqualung with the details of my failure and I am glad they at least posted the notice. I now am...

Aqualung Legend LX First Stage Failure at depth

I realize this thread is forever old, but to close this out. Aqualung has issued a consumer safety notice - I assume based on the information I sent them - this was the stated cause of my first stage failure at depth (both service center and Aqualung). Just saw this today and wanted to close...

Aqualung Legend LX First Stage Failure at depth

Well - here is the response from Aqualung - the company, not my dive shop. I sent a description of the incident via certified letter and this is the response: "It is possible for any regulator retaining nut to come loose if it is not torqued correctly, especially if the yoke screw is...

From Aquaung site FAQ SERVICE AND REPAIR (GENERAL) Q: Where can I get my Aqua Lung, SeaQuest, Apeks or U.S. Divers product serviced? We have many Authorized Aqua Lung Dealers around the country that can service your equipment. You can use the Dealer Locator function in the left side navigation...

Aqualung Legend LX First Stage Failure at depth

I took a trip to the dive shop that serviced my regulator post failure this afternoon. The service manager was there and he took the time to explain to me why he believes what we have been discussing in this thread could cause the out of air situation. There were 2 things loose on my regulator...

Aqualung Legend LX First Stage Failure at depth

I rented a tank with 1500 PSI. Put the reg on it and breathed for a while. All worked fine. Then I slowly backed the yoke screw out a bit at a time loosening the regulator to tank tightness. Breathed for a while, backed off a bit more. I repeated this several times until I started to get a...

Aqualung Legend LX First Stage Failure at depth

I guess a dive shop or Aqualung could pretty easily confirm or disprove the theory. Try 3 different tank pressures - 3K, 2K, 1K. Loosen the yoke nut a half turn, pull the first stage body away from the tank, breathe, see if air flows or not. Back the nut another half turn and try again. I...

I think you are right but I think the issue is that the yoke nut was loose and backed out enough to not push the ACD in sufficiently. That is in keeping with what the dive shop said. I think I posted my conclusion as you were posting this.

I believe I do see what the dive shop is referring to and why it would cause the issue. In the attached picture of my reg you can see the ACD and the yoke nut. With my fingers I can push the ACD in about 1/2 of what you see and this allows the air to flow. That is a very small distance. If...

Aqualung Legend LX First Stage Failure at depth

The short answer is I have no idea. I have attached a file with a side-by-side drawing of yoke style ACD closed and ACD open. Looks like the tank mating o-ring and surface pushes the ACD back against spring pressure so you get air. I see what looks like 2 o-rings in the ACD device and nothing...

Aqualung Legend LX First Stage Failure at depth

I think you (Diving Dubai) point directly to my biggest disappointment in this whole event. Today - I do not understand the precise failure mechanism and no one in the post-failure efforts really seems overly concerned and/or willing to provide me with that information. I spent the day...

Aqualung Legend LX First Stage Failure at depth

I know this is all hard to believe. I really don't believe it. I have attached the air profile from my dive computer (15 sec sample rate). We were stationary most of the dive filming small things in the sand. I was not breathing hard at all for the entire dive nor did we move far at all...

Thank you again. I just mailed the certified letter as recommended and I have the entire history of the regulator from purchase receipt, service and all emails on the issue. Best wishes.
