When I took my open water class in the 80's we had tables to 200'. My instructor gave us cards for PADI, NAUI, YMCA, and we could get a CMAS card for extra. As long as we stayed out of deco, we were good to go. I don't know how you could stay out of deco and hit 200' though. I don't remember...
I have over breathed my Meg several times. A co2 monitor would do no good. Co2 hit can happen in seconds...Best thing is to learn how to breath on a rebreather.
Sorry, looked up my old logbook. Nitrox was measuring 34% and there was a guy solo cave diving that didn't make it. Water levels were up. The next day gas measured 30%.
There are others that you don't hear about. A friend of mine was injured pumping 32% in his garage when his compressor blew. He didn't want it publicly known that it happened.
Lots of wrecks on the other side of the lake also, but no charters. And all the good ones are deeper.

At that time( about ten years ago), I was cave and mix trained thru IANTD in 2000. Previous training was padi, naui, ymca in the 80's. That was the only time I had to d & d and it was in a pool.
I dive a rebreather now (since 2007) so the entrapment, running out of gas isn't my...
Once. Went into a wreck without a line.(Never do that...) I had used a line several times before and decided to go without as there are several line traps that are a pain. Never had trouble with silt before, so I wasn't worried about it. So of course I ended up dropping a fin and silting it out...
I have flown after diving. It was a relatively shallow dive for what we normally do here in the Great Lakes. 100'. Kept it around 2500' max for an hour flight home. I am planning on flying and diving this summer up in Superior and that/those dives will be in the 200-300' range. I will wait a day...
I had this happen to me in November in Bonaire. Jetstreams. End of day 2nd day of diving, fever, flu like, skipped dinner, went to bed early. Woke up feeling fine. Went diving. End of day same thing. Fine the rest of the week.
