Many of us are concerned and looking for answers about the recent dive boat tragedy off California.

Although not about diving, this is a book that may help us understand what it's like to face a similar catastrophe.
It also highlights how small oversights, interior design miscalculations, and moments of questionable judgement, can lead to consequences exponentially more serious than anyone could have ever imagined.

It's heavy subject matter, but thought provoking.
I've read it, and I recommend it to resilient readers.
