I don't think that this has been published... Please take into account this came form PADI International (aka Bristol) so any addresses and phone numbers contained are for the UK:

PADI Americas recently discovered printer errors in certain Recreational Dive Planners (RDPs) distributed worldwide by PADI Offices. The misprints may occur in six versions of the Recreational Dive Planner (English language only). PADI Americas' printer is working diligently with PADI Americas to correct these errors.

These tables may be included in Open Water and Enriched Air Crew Paks, book and table combinations, CD-ROM combinations and ala carte tables shipped during the latter part of 2002. Please note that not all RDPs you may have received during this period contain printer errors.

PADI initiated the recall to PADI Americas and PADI Canada last week but today, erring on the side of caution, has decided to expand the recall to PADI territories worldwide.

While analysis of the problems shows that the risk of injury from the error is remote and no injuries have been reported, for the sake of safety, we feel a broad approach to this issue should be taken. We ask that you immediately check your existing inventory and remove any RDPs you received with the product numbers and production codes listed below. All RDPs included in this recall bear the printing mark "Printed in the USA" located on the back of the table, upper right corner. The product number and production code is generally located on the lower right corner, also on the back of the table. The product number and production code consists of a series of numbers and letters resembling: Product No. 60049 010PDJ2.

In checking the production code for inaccurate tables, please ignore the first three digits and only check the last four. The product number and production code may be transposed on some tables. There may also be a revision date (Rev 10/02) and/or version number (Version 1.0) listed. These numbers are inconsequential.

If you have RDPs bearing these product numbers and production codes please remove them from your inventory immediately and contact your PADI Sales Representative. Upon notification from you we will send out replacement RDPs.

To assist you in notifying your customers who may have purchased the misprinted RDPs we will supply you with product recall notices to posted in your Dive Center/Resort. A recall notice is also being sent to consumers certified after May 2002 via the PADI web site, e-mail and other means. Consumers will be directed to stop using these tables immediately and return them to any PADI Dive Center/Resort for a free replacement. PADI will replace any tables you exchange at no cost to you. We ask for your assistance by including this information in your newsletters and other correspondence that you have with your customers.


We apologize for any inconvenience this may cause you and your customers. Please contact PADI International Limited at 44 117 300 7234 if you have any questions regarding this notice.

It's true - we've gotten emails on it in the US as well. There was another thread on this board somewhere about it.

I talked to my instructor too and he confirmed the whole deal, but it's only effecting tables from October or something like that.

It's not supposed to be too bad of a misprint, but still...better safe than sorry.

However, PADI has sent out an update since then (updates in RED):

similar to the ones listed below. Only the tables with the following
production codes are included in this recall. In addition, if the dive
table does not contain a production code it is not included in this recall.

right corner on the back side of the table. The product number and
production code consist of a series of numbers and letters resembling
Product No. 60049 XXXPDJ2. The product number and production code may be
transposed on some tables.

In checking the following production codes on your tables, ignore the first
three digits and only check the last four.

Call me cynical but I have a couple of comments to make about this:

1) PADI says that the risk of injury is minor. I don't accept that at face value. The reason being that the truth (if it were to be incriminating) might end up costing them millions if someone got bent. Without knowing the nature of the mis-prints, I'm (perhaps cynically) gussing that PADI is running scared because they don't want too many people analysing the accident stats since these tables came out.

2) No injuries have been reported. I believe that PADI would say this regardless of the truth. I don't blame PADI for it but it's an American company and they are not going to come out and say "some divers have been bent using our tables, please form a queue to sue our asses off". Denyal is basic liability defense #1. Furthermore admitting it has happened will only encourage some people to make bogus claims, which will increase their legal costs enormously. They have no choice but to say this and we will never know the truth.

Just thinking out loud.
