We train over and over for failure like that but luckily knock on wood I've never had to truly experience it. Is it obvious which post it's coming out with the upstream opv since the reg won't be free flowing in your face like others?

it is less obvious than a reg freeflowing in your mouth, but again, it all depends on exposure protection. Thick hood on? Less obvious than without a hood.
