wow...that's incredible.....tragic incident.
 
draining a scuba tank through a burst disk seems risky at best....
 
Very sad, but true Darwinism in action.
There are a few ways to unstick the seat on a valve safely, but completely unscrewing the burst disk on a full tank takes the cake.

Compressed gas has huge amount of energy stored into it.

That's not what happened. The plug that holds the burst dick in place is soft brass and the nut head broke off which removed the 3 way venting holes so the gas didn't escape in a balanced way. Once it started jetting out directionally the cylinder took off. PSI actually recommends "loosening the burst disk" to confirm stuck valves have vented so its not like they did something crazy stupid here. Strapping the cylinder down was the missing element.

My understanding is the entire plug came out by accident. The air from the burst disk assembly orifice on the valve caused the tank to bounce around until it smacked and broke the valve off. After that it was a rocket and is when it hit the fireman in the head.
