Here’s all the parts after taking the valve apart, seems I’m missing the insert house portion and maybe a metal washer under the lock nut.

So far what I found was the demand valve body was missing an O-Ring. I wouldn’t think this would cause an internal valve leak and it wasn't leaking on the outside around the hose.
Correct, that o-ring is a case seal and if missing might allow water into the case or perhaps would cause a vacuum test to fail.

Sorry I don't have a better diagram-I'll attach an annotated copy with the o-ring sizes for your HP. The parts I mentioned earlier are numbers 28 (insert) and 30 (washer.) If they are made of plastic or phenolic they are very subject to wear and could cause sloppy movement of the valve mechanism. If item 30 (washer) is metal, ensure the rounded face is toward the lever.

Before you disassemble any further, remove the seat and flip it over (if you don't have a new one readily available) and give it another try as another data point.

Mine has the (plastic/phenolic hard to tell the difference it is brown) round washer under the lock nut, didn’t find an insert, do you have a picture of what it looks like hard to tell in the diagram.

Sorry, I don't have a picture, but I think I can see your 108 has a square broach that the poppet stem goes through. Therefore an insert is not required. However, let's make sure. Post a picture that shows the inside of the case-focus on the hole where the poppet stem exits the air barrel. It will either have a square broach stamped into the metal or a brown (or white) insert.

That would clear that missing part up, thanks. Strange the stem would run metal to metal. Here’s the pic.

I have 3 108s, all have the square bore in the valve body with no insert.
