Usually but I normally have a camera in hand so I have video lights or focus lights available to peer into spaces if needed.
 
At one time, I owned several, and frequently carried a small one attached to my camera rig. I am changing over to digital now and will have a video light to serve for fill light as well as to show the way. I guess that means I’ll have to check “Always” from now on.

Never. If I'm going to dive in the dark (night, caves, wrecks) i feel that it is important to experience the dark as it was meant to be. Without a light.
Wow!! if you dive in the dark without light, it could be difficult to see clearly around you.
