Largest as is size (small, medium, large), or largest as in lift?
Well, a BP/W can be made to fit the largest diver and you can put an extremely large wing on it.

I'm all for everyone enjoying diving, but if you need to use a BCD that large, perhaps you should consult with your doctor first. Perhaps the best thing to do for your health would be to work on your cardiovascular system first before putting yourself at risk.
