Any way to contact the instructor or former dive shop owner? Your card has to be somewhere, perhaps someone still has it?
While I wouldn't expect them to cut me in slack in this situation, it can't hurt to try. Just be aware that you were given ample time to get the card and did not. In the end, the responsibility was yours.
