Question Oceanic Slimline 2 started leaking after service. Did they screw up or did it fail?

Greetings! I brought an Oceanic FDX10, Delta 4, and Slimline 2 in for service. They called me to say they can't get the Slimline 2 to stop leaking after servicing it. They said they used two service kits but can't get it to stop. They said they'd try again next week. Next week comes and they call to say they're super frustrated with it and done trying to fix the leak. They offered to sell me a new octo at a discount. Has anyone ever had this happen to them? Is there some known issue or common mistake that could have been made? I remember reading somewhere that it's possible to over-tighten something during a repair and that you have to replace the part if that happens per the Oceanic tech training. I have no idea if that would apply to this situation though. Thanks in advance.
