It happened to my roommate in Cozumel with Dressel divers. He had to go see a Mexican md to get cleared. One of the top cause of death in men is "heart attack while swimming".
It isn't unreasonable to request clearance. I wouldn't want to burden anyone with witnessing or trying to prevent my death.
I'm only 54 but seeing this requirement got me back on track with my annual physicals. The dive industry succeeds through self regulation and should therefore constantly evolve. I don't fault them for erring on safety's side. You should be made aware of this at booking time IMHO.
