The danger is going over by a little bit, that was OK, well go a little more, that was ok etc etc.

There is a good video (4 parts) on Normalised Deviation. Its about the NASA failures. Quite a good video to watch and exactly about accepting deviation from the norm as norm and then deviating from that until you reach the point of no return.

I know we have bursting discs fitted (don't we?). Well I found a set of doubles I recently purchased with double bursting discs in each valve, so much for the relief at max pressure! The only reason I looked was that one was leaking so I drained it and opened it up. The tank you have may not be as safe as you think. While this situation is clearly not the norm, its not the normal situation that kills you!
