I don't know whether clothing errors count here, but earlier this week on holiday in the red sea after completing a dive I removed my wetsuit. And in the process it ripped the shorts I was wearing almost in half - via a line straight down the back of them.
The trouble is I didn't notice this...
Well OK I definitely won't try diving for it.

Edit: Well I've gone back to the pier and climbed down the ladder, and someone's been down there and managed to semi-retrieve it!
Although it's covered with seaweed I've lifted it out and got it back.

In fact the rope is still attached to the anchor, and I was able to tie the end of it to a ladder on the pier leading down to the water. Then later I walked onto the pier and tried to pull the rope in, but it was stuck fast, pointing directly downwards suggesting that it was being held by...
For reasons I won't go into, I had to abandon a stuck small anchor + chain + rope next to the north side of the fishing pier at the entrance to Langstone Harbour near Portsmouth (UK).
Although it is probably uneconomic for me to recover it, as I have just passed my PADI open water certificate it...
Yes I think that was what happened with me. When I experienced the ear pain first of all during the dive at about 3 meters I rose to 2 meters to try to equalise, but failed. Instead of trying again and again I should have risen further, possibly to the surface.
I decided not to dive today...
I had my first open-water dives today as part of my PADI OW course. The first dive was fine - down to 5 meters. The second one was also fine - and I stayed there for 15 minutes before returning to the surface with the instructors.
